|UCI Eventive Venue Request System||UCI Eventive is an event inquiry system for requesting Student Center spaces such as conference rooms, Ring Mall, outdoor plazas, and Aldrich Park (restrictions may apply).|
|General Assignment Classroom Inquiry||Survey to request General Assignment Classroom (GAC) and lecture hall space.|
|Zoom Event Inquiry and Planner Meeting Request for Registered Campus Organizations||Registered campus organizations can host their own Zoom webinars and events. Student Center & Event Services can work with you to set up a Zoom webinar with up to 3,000 participants.|

|Request for Sound||Sound ExceptionRequest to have sound at your outdoor event. Submit this completed form to your SCES event planner.|
|Temporary Food Permit Application||Registered Campus Organizations must complete and display a temporary food permit if they are serving perishable food.|
|Request to Serve or Sell Alcoholic Beverages||Request to have alcohol at your event. May also be referred to as an alcohol permit. Submit this completed form to your SCES Planner.|
|Additional Approval for Catered Events (University Policy BUS-79)||University business meetings or events associated with a UC campus with catering may require a Signature of Approving Authority as Additional Approval. Submit this completed form to your SCES Planner.|
|Short-Term, Temporary Custody or Storage of Property||If you plan to store equipment and personal belongings onsite at the Student Center, you will need to fill out this form. May also be referred to as a storage waiver. Email the completed form to your SCES event planner.|
|Event Liability Waiver Information Form||Form for SCES event clients whose events involve high risk elements (e.g., minors) to request liability waivers from UCI Risk Services.|
|Request for Free-Standing, Temporary Structure||Organizations wishing to construct a free-standing, temporary structure or display (including Greek letters) for their event must submit this request to their Senior Meeting & Event Planner.|

|Inquiry for AntMedia Videography & Photography Services||AntMedia provides flexible and customizable services to fit your event and project needs.|
|Application for the UCI Film and Photography Permit||Request to film or photograph on campus property. RCOs, UCI students (non-course coded class film projects), UC departments, and off-campus entities should complete this application and email it to firstname.lastname@example.org with a script, storyboard, or outline.|
|Temporary Vendor Permit Application and Temporary Vendor Agreement||Off-campus vendors who are interested in selling merchandise on campus must apply for a Temporary Vendor Permit. They will need to complete these two (2) forms and submit them for review and approval along with a certificate of Commercial General Liability insurance.
Please note: the vendor must have a valid California resale license and is required to collect and pay the prevailing state sales tax and the vendor must have a UCI departmental sponsor.
|Infraction Appeal Request||An individual or organization who receives an infraction may appeal in writing within five days of receiving notification of loss of privileges. Submit the completed form to email@example.com.|
